\nAs a Health Coach Nurse, will help patient console and care. Inspire patients to engage in their health. Provide tools for healthy alternatives and habits.
\nWhat You'll Do
\nAs a Health Coach Registered Nurse,
\n\nYou will assess client recommendations for patient populations.\nYou will evaluate health coach interventions and respond to medical referrals. Your goal is to identify patients meeting care criteria.\nYou will partner with teams and partners.\nYou will involve patients in care and execute personalized care plans.\nHelp identify and address barriers to accessing services.\nYou will use communication skills to interact with patients in person, by phone, or digitally.\nEnsure patients receive necessary services through reminders, medication reconciliation, and reassessments.\n\nWhat Will Make You Successful In The Role
\n\nTo succeed as a Health Coach RN, you need knowledge of nursing guidelines and patient care.\nInterpersonal, communication, problem-solving, and critical thinking skills are crucial.\nFamiliarity with basic computer skills is also required.\n\nBelonging Statement

\nQualifications
\n\nEducational Background: Possess an Associate's Degree in Nursing.\nWork Experience: Bring at least two years of relevant professional experience.\nCertification: Hold a current and valid registration as a Nurse.\n
