**Req Number** 147724 Job Description Assists in the maintenance, planning and implementation of the department's programs and services. Organizes, coordinates and implements programs and events to educate, develop and foster public interest, understanding and good will in the community. Job Responsibility + Develops and presents health education programs in the form of workshops, and individual sessions + Provide HIV/STI risk reduction and counseling, and treatment adherence education. + Plans and implements programs including, but not limited to fairs, seminars, training programs, conferences, workshops and public forums. + Assists with care coordination, outreach, and follow up to retain patients in care. + Provides assistance to patients in overcoming Social Determinant of Health barriers. + Collaborates with care team to determine health needs and the availability of services, and to develop goals for meeting needs + Documents activities, recording information such as patient interactions, intakes, assessments, and service plans. + Develops and maintains a cooperative relationship with coworkers, community-based organizations and agencies. + Prepares and distributes health education materials, including reports, bulletins, and visual aids. + Assists in timely deliverables of Grant related tasks + Maintains policy and procedures that meet the needs of the system and the department; keeps staff apprised of relevant information. + Performs related duties as required. Job Qualification + Master's Degree preferred in health education, public health, community health or related fields or equivalent combination of education and related experience. + 1-3 years of relevant experience, required. *Additional Salary Detail The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future. When determining a team member's base salary and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity).
