Position classification
Hybrid -- Job duties and expectations allow for onsite and remote work scheduled every week. Employees in this classification are regularly scheduled onsite one (1) to four (4) days per week based on the County needs and as determined by Department Director. Hybrid classified roles can be onsite more than the set minimum based on employee preference.

What Success Looks Like In This Job
The Health Educator is a public health professional, with lived experience, who is an advocate for their community. This position works to promote, maintain, and improve the health of their community by providing individualized support to people within their community and fostering relationships with organizations. This position will work closely with community members to provide screening and address socioeconomic barriers for behavior changes and client resource navigation, assist in the team’s collaboration with community agencies, facilitate programming for community members, and provide outreach activities in the community.
This is a temporary full time position that is authorized to work for up to 12 months. This position is eligible for Medical coverage for themselves and children (no spousal coverage) and the 457b deferred compensation plan through Nationwide. This position is not eligible for any other benefits including dental, vison, holiday/sick/vacation accruals, and pension participation.
The anticipated hiring range for this role is $22.969/hour - $26.414/hour. The full salary range for this role is $22.969/hour - $32.156/hour.
Examples of Duties for Success
Health Educator (INSIPRE-CVH) (50% of role):
+ Provide CVD health screenings and follow-ups with clients. Complete follow-ups, health coaching, quarterly check-ins and retesting as appropriate.
+ Identify clients at-risk for cardiovascular disease (CVD) and its risk factors, focusing on the underserved population.
+ Use motivational interviewing techniques to strengthen the client’s own motivation and commitment to change. Assess client needs, concerns, and readiness to change.
+ Offer culturally- and literacy-appropriate CVD education and health recommendations provided by the Outreach, Screening and Referral (OSCAR) data system.
+ Provide referrals to medical care, behavioral health care, healthy living resources, and social services for at-risk patients. Help clients find additional information from local resources and/or credible websites.
+ Create and maintain library of appropriate education materials approved by CPC.
+ Create and maintain directory for local health care providers, community resources and services, especially free or low-cost services.
+ Develop relationships with health care providers, eligibility workers and community programs.
+ Use the OSCAR data system in real time with clients, accurately documenting client information, goals/action plans and INSPIRE-CVH services provided.
+ Participate in quality review and evaluation of INSPIRE-CVH data. Synchronize OSCAR data to the INSPIRE-CVH database at least weekly.
+ Attend all CHW trainings, meetings and conference calls as directed.
National Diabetes Prevention Program (NDPP) (30% of role):
+ Hold a National DPP Lifestyle Coach Certification or complete certification through a CDC approved National DPP Lifestyle Coach training entity within 4 months of hire date. Adams County Health Department will pay fees for Certification Course.
+ Complete at least two hours of Advanced NDPP Lifestyle Coach Training each year.
+ After successful completion of Lifestyle Coach training, facilitates classes using CDC recognized NDPP curriculum and related activities including handling of logistics for both in-person class cohorts held in community settings and virtual class delivery.
+ Consults with the assigned Diabetes Educator RDN regarding high-risk clients and situations, medical problems or change in client status or related issues. Assist in preparing and reviewing educational materials for clients.
Diabetes Education and Prevention Team (20% of role):
+ Collaboratively works to assist Diabetes Education and Prevention Program team to meet program implementation plan goals, to obtain necessary clinical information for delivering NDPP services, and to communicate with providers or community resources appropriately.
+ Screen for socioeconomic barriers to behavior changes and link clients to appropriate community resources.
+ Delivers services through in-person visits, telephonic counseling, case management, telehealth, and community outreach programs.
+ Participates in Diabetes Education and Prevention Program meetings and other applicable meetings as assigned.
+ Participates in community outreach activities such as health fairs, health coalitions, and workgroups.
+ Supports Program in the development and maintenance of positive working relationships with community-based organizations.
+ Performs other duties as assigned and required.
Qualifications for Success

Education and Experience:
+ High School Diploma or equivalent and at least 2 years of experience in one or more of the following areas: community involvement, working directly with individuals, serving as a community health worker or patient navigator, or volunteering/employment with a community, health, or social service organization.

+ Associate’s Degree and at least 1 year of experience in one or more of the following areas: community involvement, working directly with individuals, or volunteering/employment with a community, health, or social service organization.
Special Qualifications:
+ Excellent verbal and culturally competent communication skills (in person, on the telephone, and through email correspondence). Bilingual (English/Spanish) skills preferred.
+ Ability to creatively tailor educational messages to a variety of populations and assess and solve problems.
+ Must possess demonstrated ability to relate to individuals of varied ethnic, cultural backgrounds, ages and economic circumstances.
+ Trusted member of the community and ability establish and maintain rapport with patients.
License and/or certification
+ Possession of a valid driver’s license and dependable transportation or the ability to use other means of transportation to work from various ACHD offices and community locations.
+ National DPP Lifestyle Coach Certification or complete certification through a CDC approved National DPP Lifestyle Coach training entity within 4 months of hire date.
Background Check
Must successfully pass pre-employment testing which includes motor vehicle record (MVR)and background check.

Equipment Used
Standard office equipment including, but not limited to, personal computer and associated hardware and software, telephone, copier, fax, and standard medical equipment such as a stadiometer, scale, and hemoglobin A1C testing.
WORKING CONDITIONS AND PHYSICAL REQUIREMENTS
+ Work is primarily conducted indoors with occasional time spent in and outside environment.
+ Incumbent experiences routine exposures and hazards of a typical office environment and the frequent noise associated with working in community settings.
+ Requires travel between ACHD clinics and community programming locations.
+ Requires working outside regular hours to attend meetings and teach classes (evenings and weekends).
+ Frequent light lifting and moving under 15 pounds; occasional moderate lifting and carrying up to 49 pounds to lift supplies.

EMERGENCY PREPAREDNESS AND RESPONSE DUTIES Employee responds, as required, to support public health emergencies, incidents and events.
