Child Health Home Coordinator position will serve as a liaison between RG’s mental Health Home Services and Preventive case managers, aiming to identify and assist families in enrolling in our Health Home Program. Secondary, the Child Health Coordinator (CHC) will support children and family members in various ways, including providing transportation, conducting home visits, and accompanying parents to service providers.

Responsibilities Attend General staff meetings to report out on number of referrals to health home per month by program. Presentations / Public SpeakingMeet with case managers, interventionists and therapists to discuss new cases that may qualify for Health home services.Develop a spreadsheet of all active cases in the preventive program. Provide monthly reports of children who meet criteria (complex trauma). referred to health home and number enrolled.Identify and refer families with children who qualify for Health home servicesBe familiar with the qualifying clinical criteria for health home enrollment.Meet with Health Home staff to ensure communication, follow up with referrals and to track the outcome of referrals.Provide brief trainings to the staff on the qualifying criteria and benefits of the health home program.Possess clinical understanding of criteria to qualify for Health Home.Attain school and medical reports; document in Connections utilizing appropriate progress note template and file in case record. Place printed copies of all important document in case files.Participate in mandatory once a week individual and unit supervision; attend all mandatory training courses, general staff meetings, as well as serve on committees as assigned.Other duties as assigned i.e. clinical or psycho education groups Qualifications Bachelor or master degree plus a minimum of two years of prior experience in child welfare.Must demonstrate strong organizational and computer skills including proficiency in Microsoft Word Outlook, Excel PROMIS and Connections.Driver’s license a plus.Bi-lingual (English and Spanish) Preferred.

 

Supervisory Responsibility: No

 

Work Environment: Brooklyn Office, with one (1) remote day every two (2) weeks.

 

Position Type/Expected Hours of Work: Full time position, scheduled appropriate to the needs of client and families. This position calls for a thirty-five (35) hour work week, but more than 35 hours per week may from time to time be required.

 

Travel (if required): Although the main location is the Brooklyn office, travel required to Rising Ground sites in the Bronx, Harlem and Queens, including home visits. Work-related travel expenses will be reimbursed by an agency.

 

Salary Bachelor's level: $50,000/ Annually.

Salary Master's level: $60,000/ Annually.
