Description
The Health Navigator (HN) is responsible for performing health insurance enrollments, outreach, and educational presentations related to the NYSOH Navigator Program. The HN is responsible for ensuring all individual monthly deliverables are met in accordance with funder requirements. The HN is responsible for ensuring timely and effective client advocacy services in the areas of insurance & health care access.
Essential Job Functions
The following duties are mandatory requirements of the job:
+ Provide healthcare education to consumers about how to use the NYS of Health website and marketplace, choose a health insurance plan, or make changes to an existing plan.
+ Assist clients to obtain all required documentation to obtain a new health insurance plan.
+ Conduct targeted educational presentations both onsite, at GMHC, at community events, private businesses, and community partners to promote the program and availability of health insurance.
+ Conduct outreach to promote the program and identify fixed sites that may benefit from health insurance enrollment assistance.
+ Document deliverables and services in applicable databases.
+ Adheres to workplace policies and procedures, including confidentiality, documentation, channels of communication, workplace culture, dress code, and conflict resolution.
+ Perform continuous quality improvement activities as per the triad management model to ensure program performance, in collaboration with program team.
Education and Certification
+ Bachelor’s degree preferred; High School Diploma required.
+ Bi-Lingual (Spanish-speaking) preferred
+ Willingness to receive navigator training and remain certified for duration of employment.

Special Skills and Knowledge
In addition to the above-listed job responsibilities and educational requirements, the ideal candidate for this position possesses most or all the following:
+ Must have extensive knowledge of the challenges and opportunities presented to PLWHA.
+ Proficiency in Microsoft Office products such as Outlook, Word, and Excel.
+ Excellent written and interpersonal communication skills.
+ Must have three or more years’ experience with social services, including government benefits.

This is a Hybrid position which requires some days in office
