This full time position will work daytime hours onsite with some WFH flexibility.

GENERAL OVERVIEW:

This job will develop and maintain relationships with EMS (Emergency Medical Services) leadership and personnel in order to support and facilitate the co-responder collaborative model, as part of the Street Medicine Urban Poverty program,

ESSENTIAL RESPONSIBILITIES

Engages with hospital Emergency Departments, substance us centers of excellence, engagement centers, certified assessment centers to track participants experiencing homelessness and ensure service connection. (20%)Builds relationships with harm reduction and drug and alcohol providers to ensure rapid connection to services and supports. (10%)Coordinates and leads case conferencing with CIH co-responder team members (EMS). (10%)Trains CIH outreach staff in understanding and navigating the healthcare system. (10%)Collaborates with AHN clinical services in Allegheny County Jail. (10%)Provides disposition planning for hospital discharge for people experiencing homelessness and those who are unstably housed across health systems. (10%)Collaborates effectively with respite staff to coordinate referral and discharge. (10%)Drives the development of complex care planning for people who are high utilizers and case conferencing for individuals experiencing homelessness. (10%)Schedules and supports clinical street outreach rounds with clinical partners. (10%)Other required duties.

QUALIFICATIONS:

Minimum

Bachelor's Degree in Social Work, Public Health, Psychology, Education or related field1 year in Public health or social servicesValid PA Drivers License

Preferred

1 year with Behavioral Health1 year with community engagement organizations1 year with hospitals and EMS respondersExperience engaging health care providers, key stakeholders and related community organizations to collaborate on connecting individuals to quality healthcare.
