Active Day has an immeidate opening for a Full-Time Healthcare Coordinator
at our Fort Wayne Center
What You Will Do:
At Active Day, our Healthcare Coordinators contribute greatly to the care and well-being of our members. You will provide coordination of care needs to members to improve their health status and quality of life as well as delay any possible deterioration of health status and manage chronic conditions. All this will is done in collaboration with the physicians, and integrated medical and social services.
Our Healthcare Coordinators are LPNs, RNs or have their MSW and therefore impeccable at ensuring that all work complies with company policies and procedures, safety and regulatory laws and standards. As Healthcare Coordinator, you will work closely with our members and their support team, which includes but is not limited to the Care Management team, Family/Caregiver, Center Director, IHCC Director and Nurse Manager within the center, as well as the member’s Treating Practitioners.

Development and oversight of a healthcare support plan that includes coordination of medical care and proactive care management of both chronic diseases and complex conditions such as recurring falls, depression and dementia. Collaboration with Treating Practitioner(s) (i.e. Physicians) on member careMember medication review Assist and/or accurately complete required admission documentation and members health record.Communicate in a positive, courteous and professional manner with members, their families/caregivers, support team and community at large at all times. Ability to travel up to 25% in the local market. QualificationsAs Healthcare Coordinator your qualifications will include:
MSW or LPN required, LPN or RN preferred. Valid malpractice insurance policy and registered with the applicable state examining board.MSW must have a minimum of two (2) years of experience providing healthcare coordination. Valid Driver’s License and Proof of Insurance/RegistrationExperience working in a health care facility, preferably working with seniors. Strong communication and engaging interpersonal skills required. Successful results of background check
