The Healthy Housing Lead Case Manager blends the mission of AHF and the Health Housing Foundation (HHF) by providing case management for residents of HHF locations.

Under the guidance of the Manager, the Health Housing Lead Case Manager will add consistent value to the program.

The HHLCM will assist clients with access to resources, work with potential and existing tenants to remove barriers by facilitating a collaborative process that identifies members’ needs, a plan to address those needs, and continuous follow-up to ensure that members obtain the necessary services that help them to maintain successful residency. The HHLCM coordinates with other service providers through a comprehensive and active referral process to link clients appropriate services and care. 

 

With a strong skill set, a caring and mission-driven focus, you will work with HHF residents, external agencies, area housing coalitions and other related resources to implement, support and monitor the housing case management needs of the residents. You will  serve as a key component in improving the resident’s  stable housing options focused on successful residency and improved quality of life.   

 

Essential Duties & Responsibilities Includes the following - Other duties may be assigned:

Assist the Manager of Care Management or designee with different administrative duties.Coordinate with Healthy Housing Foundation team for understanding of needed actions, trends, immediate concerns and access specific to HHF locationsParticipate in collaborating with and establishing strong communication lines with location specific service providers and designated continuum of care including the use of software systems ( i.e. Homeless Management Information Systems, Housing Forward, All Neighbors Coalition).Assess, collect, and maintain accurate member information and records in a confidential manner.Develop/access a comprehensive database of resources designed to address client needs including Initiatives or Projects that incorporate wrap-around services that are equitable, inclusive, and tailored to the needs of individuals and families; including but not limited to (i.e. food banks, legal aid, employment opportunities, healthcare)Educate members about various community resources and programs to increase their knowledge of appropriate referralsDevelop clearly written individualized service plans to promote self-sufficiency and resolve barriers to overall stability including permanent housing stability, income and employment barriers. Assess, verify and maintain records of Case Management and various entitlement programs Maintain accurate documentation in all stages of the housing program in specific data systems. Documentation will be completed within 72 business hours of encounter, including case notes.  Complete monthly reporting to ensure accurate programmatic reporting (based on position/site assigned)Provide active referrals and application assistance and support to membersEducate members about various community resources and programs to increase their knowledge of appropriate referralsAttend housing coalition agencies, advocacy groups, etc identified and or appropriate to HHCM’s assigned location in order to inform clients on various resource opportunities.Communicate regularly with staff about changes, updates, and improvements in service delivery issues or other agency related issues that directly or indirectly impact staff and/or membersAttend agency, local, state, and national meetings, conferences, or workshops as needed or requiredAddress members concerns utilizing the Customer Service StandardsPerform routine self-audits and maintain charts to ensure members information and eligibility documents are currentMeet with supervisor at least on a monthly basis to staff member cases, discuss chart audits/observation results, and to discuss performance and progress or stated goalsAttend all required internal and external (for off-sites) meetingsParticipate in various trainings held internally and externally ensuring a continuous knowledge base of programs that benefit members.

 

Qualifications

Education and/or ExperienceBS/BA in Housing Case Management, Social Work, or other social service field with two (2) years of direct service deliveryFamiliarity with housing entitlement systems and proceduresFamiliarity with available resources in assigned localeAbility to work effectively in a team environment with minimal supervisionOther Skills & Abilities/Qualifications

Requires one-year experience as an HIV-testing counselor.

Excellent organizational skills.

Ability to work with minimal supervision.

Must have reliable transportation and proof of insurance.

Skill in communicating openly without prejudice or judgment.

Ability to communicate clearly, effectively and in a sensitive manner.

Ability to work well in high stress environment

Very strong interpersonal communication skills with understanding of de-escalation strategies

 

Language SkillsBi-lingual Spanish preferred.

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.  Ability to write routine reports and correspondence.  Ability to speak effectively and present information and respond to questions from groups of managers, clients, customers and the general public.

Certificates, Licenses and Registrations

Valid driver’s license in state of employment, with proof of insurance

TravelMinimal travel 10% from housing unit to area resources, agencies, healthcare centers in assigned area. AHF Commitment
