Pay Rate: The pay range for this role is generally $46,300-$51,800 annually, commensurate with relevant experience and qualifications and in alignment with internal equity. Community Health Action of Staten Island (CHASI)drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI serves the most vulnerable individuals, families, and communities with critical services and programs. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – the poor and working poor, low-income people with chronic illnesses, persons with criminal justice involvement, substance users, domestic violence survivors, people of color, and the LGBTQ community.
POSITION SUMMARY:
The Prevention & Intervention department provides comprehensive HIV, STI, and Health Equity prevention services. The Healthy Relationships Counselor is responsible for facilitating short-term Healthy Relationships groups for individuals who are HIV positive, Peer Program Training for high-risk women, and enrolling high risk and/or HIV positive women in HIV Navigation Services for medical health, behavioral health, and social service needs. The individual will also collaborate with internal and external contacts to recruit for the program.
DUTIES & RESPONSIBILITIES:
+ Recruit women for the various components of the program: Healthy Relationship Group, HIV and Hepatitis C testing/screening, Peer Training Program, and HIV Navigation Services (HNS). Develop internal and external collaborative relationships and conduct outreach to recruit for these program areas.
+ Conduct behavioral risk and intake screenings and enroll clients into HNS and link clients to service needs that are identified.
+ Facilitate groups for program participants on an on-going basis.
+ Develop service plans to ensure clients are linked to identified services, including but not limited to medical, mental health, harm reduction, substance use treatment, health insurance, SNAP benefits, shelter assistance, HASA advocacy, legal services, HIV and HEP C testing.
+ Perform follow up to ensure the linkages / referrals have been completed.
+ Supervise volunteer peer outreach workers assigned to assist with group facilitation, outreach, and linkage and navigation services.
+ Train peers to co-facilitate Healthy Relationships and Peer Program.
+ Prepare program reports as required.
+ Conduct HIV and HEP C testing.
+ Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values.
+ Other duties as assigned.
Requirements
QUALIFICATIONS:
+ At least 2 years of related experience in HIV prevention and/or working with individuals who are HIV positive and linking clients to services.
+ HS diploma or equivalency; college degree or some college preferred and can be considered in lieu of 1 year of experience.
+ Group facilitation experience preferred.
+ Bi-Lingual (Spanish/English) highly preferred.
+ Demonstrated ability to independently manager work and/or program management experience.
+ Knowledge of community resources and organizations on Staten Island preferred.
+ Ability to juggle priorities and manage time independently and productively; thrives in fast-paced environment.
+ Effective oral and written communication skills.
+ Proficient in Microsoft Word, PowerPoint and Excel and internet navigation.
+ Demonstrated ability and/ or commitment to working effectively with clients from diverse backgrounds, with sensitivity to ethnic and cultural backgrounds.
+ Flexible, able to change gears at a moment’s notice and “go with the flow” to address multiple priorities.
+ Highly organized, strong attention to detail, and ability to follow strict protocols.
+ A team player with an ability to work on diverse teams with shared responsibility and accountability; ability to contribute to a positive, productive, open, and supportive environment.
