Children's Outpatient Services provides treatment for children and adolescents and their families and is intended to improve and maintain a child’s or an adolescent’s stability in the community. Treatment focuses on problem resolution, symptom reduction, stabilization and/or prevention of movement to a higher level of care. Services include individual, family and group therapy, medication evaluation and management, case management, in home intervention, case consultation and summer day treatment services. The type of treatment is highly variable and depends upon the individual’s existing symptoms and developmental level.

Responsibilities

Primary Job Functions:

 

Care coordination Responsibilities (Staff to consumer ratio will not exceed 1:18)

Completes a comprehensive home-based assessment of the consumer’s needs and strengths utilizing the CANS within 14 days of intakeIdentifies goals and plans for appropriate services based on the CANSDevelops Individualized Service  Plan (ISP) within 30 of first face-to-face meetingIntensively collaborates with the treatment team members and outside sources in the delivery of care coordinationProvides appropriate linkage to community resourcesProvides ongoing monitoring of consumers’ progress and needs as assessed in the CANSAdvocates on behalf of the consumers to secure community resourcesEvaluates the consumer for discharge planningEffectively demonstrates the application of child/adolescent specific competencies

Case Load Management:

Effectively manages caseload based on consumer needsMeets AltaPointe productivity standards

Supervision and Consultation

Seeks clinical supervision and consultation as neededAccepts and employs suggestions for improvementActively works to enhance knowledge and skills

Clinical Record Keeping

Documents in a timely fashion per AltaPointe policyDocuments appropriate, complete, and comprehensive CANS assessmentsDocuments measurable and specific goals in the individualized service planDocuments in a clear and concise manner, types of consumer problems, ISP goals addressed and services providedDocuments case disposition, case closings, follow-up and referralsDocuments legibly

Courteous and respectful attitudes towards consumers, visitors and co-workers

Administrative and Other Related Duties as Assigned:

Bachelor’s degree preferred in Social Work (BSW). Hospital Case Management and/or discharge planning experience preferred.

Must be 23 years of age or older with a valid Alabama Driver’s License and proof of current liability insurance.

 

Knowledge in adult specialty, outlined below, to be attained within (6) months of employment:
