HIIM Data Integrity Quality Analyst - Auditor (Remote)
Job Description

Essential Duties and Responsibilities:

Performs quality audits of the electronic record quality and integrity based on CHS policy and procedure as well as federal and state regulatory guidelines.Ensures integrity of the legal medical record.Each week, randomly selects a sample of discharged, analyzed encounters to verify all documents both electronically fed and scanned are captured in the electronic medical record.Performs Adjust Document Index functions via McKesson Patient Folder.Performs Image Modification in Cerner Millennium.Verifies correct indexing of the MRN, account number, and/or document type.Verifies quality and correctness of all scanned images from the imaging vendor and the facility.Identifies documents needing to be re-scanned due to poor image quality and routes those documents to the appropriate work queue for processing.Corrects errors noted while auditing and provide feedback to the HIM Data Integrity Supervisor on a weekly basis.Documents all audits performed on the Weekly Monitoring Log.Tracks major and minor errors for trending.Communicates via established protocols with the document imaging area.Serves as a backup to the Centralized Data Integrity Analysts.Addresses appropriate questions related to medical record completion from the HIIM Data Integrity Manager, HIIM Data Integrity Supervisors, Facility HIMDs, Physicians and/or other providers.Assists with maintaining Cerner Batch Indexing (Cer Batch) queueAssists with maintaining Inbound Summary TOC Accounts through the deficiency management’s Message CenterAssists with monitoring designated facility reports via Discern Analytics to include but not limited to:Anticipated History & PhysicalAnticipated Discharge SummaryAnticipated Operative ReportsPhysician Refuse to Sign ReportContributor System ReportOthers as directedMeets or exceed corporate established Medium productivity standards.Meets or exceeds corporate quality standard of 95% or greater on all audited medical records.Assists in maintaining all equipment and supplies required to provide services in an optimal manner.Promptly reports issues and trends not complying with facility or corporate policies/standards.Assists in cross-training other employees as necessary.Able to work independently as a remote employee.Ability to work a schedule that may include weekend coverage with Saturday and/or Sunday.Demonstrates competency of all Audited assignments within 90 days of hireRequires that employee sustain a secure home working environmentRequires that employee sustain a secure home internet service and routerRequires that employee ensure company issued equipment stays maintained and intactPerforms other duties as assigned.

Qualifications:
Required Education: High School Diploma required

Required Experience:

Three (3) years’ experience in medical record analysis requiredOne (1) year experience with medical record modifications and auditing required

Preferred Experience:

Previous experience in an HIM Department or office environment strongly preferredExperience with electronic medical record systems - McKesson Horizon Patient Folder and Cerner MillenniumAnalytical skills with the ability to problem solve and identify potential workflow barriers/issuesMust be able to work independently as a remote employee and prioritize multiple tasksAbility to decipher physician handwriting.Possess strong written and verbal communication skills

Computer Skills Required:
To perform this job successfully, an individual should have knowledge of Horizon Patient Folder (HPF), Transcription platform software, Google Suite, Microsoft Office applications; Database software, Audio/Video Conferencing
Applicant must demonstrate proficiency in use of keyboarding and use of a mouse.
