HIM Release of Information Associate

Position Overview:

Professionally represents Capital Health Mission Statement by answering phones and greeting in person medical record requestors promptly and courteously. Answers questions related to release of information both in person and over the phone. Uses various hospital systems to search patient information to complete medical record requests.Prioritizes record requests and communicates with other medical facilities and Capital Health departments to assure that information is received as soon as possible by requesting facility or department within Capital Health.Performs self-quality checks to successfully release a complete and accurate medical record, as needed for but not limited to, patient care at Capital Health and all other medical facilities. Gathers data from several systems simultaneously and assure that all are delivered together to requestor timely and accurately and generate invoices of services.Gathers a "complete" record using multiple EMR systems, offsite storage, outpatient EMR's and hard copy.Responds to Risk Management medical record requests timely and accurately. Electronically tracks all charts located in our risk lock-up area and updates database as necessary. Communicates regularly with Risk Management staff to assure requests are completed within strict timelines according to their legal requirements.Use hospital master patient index to look up patient information to validate medical record requests. Provide feedback to requestors for invalid medical record requests.Promotes quality improvement, staff and patient safety by validating requests for records and ensuring that accurate records are released as requested and adhering to minimum necessary standard.Maintains current knowledge related to applicable statutes, regulations, guidelines and standards necessary to perform job duties in accordance with the requirements of our organizational policies.Provides excellent customer service by being attentive and respectful; insures understanding of customer request and follows through as promised; and being proactive in identifying requestor concerns or problems. Helps resolve customer service matters in a polite manner.In compliance with hospital policy regarding faxing, faxing software is used whenever possible to reduce the risk of dialing errors. All personnel must use the HIM department fax cover sheet with every fax transmission.Performs other duties as needed.

MINIMUM REQUIREMENTSEducation: High School Diploma or equivalency. Knowledge of all state and federal release of information regulatory requirements. RHIT or RHIA preferred.Experience: One year of previous experience in release of information.Knowledge and Skills: Proficient knowledge of all state and federal regulatory requirements. Proficient use of multiple disparate EMR systems.Special Training: Proficient skills in Word, Excel and email required. Bi-lingual preferred.Mental, Behavioral and Emotional Abilities: Ability to work alone or as a team member showing positive outcomes in specific timeframes. Strong customer service skills, ability to work in sometimes stressful environment. Perform detail-oriented tasks with conscientious awareness of detail and accuracy.Usual Work Day: 8 Hours
