SUMMARY (BASIC PURPOSE OF THE JOB)Performs a variety of data management functions for the support of the HIM Department. Collects, organizes and reviews data on a routine basis. Presents results in a logical and organized format. Prepares monthly statistical information for the hospital accreditation body. Assures that the transcription processing is accurate and timely by consistent review, monitoring, compiling data and performing any necessary revisions across interfaces. Assures Continuity of Care information is disseminated to next provider of care. Uses several electronic hospital EMR's to analyze and assign documentation deficiencies to facilitate accurate completion in accordance with Capital Health, hospital accreditation body, CMS and Department of Health regulations. Performs other patient care release of information and management support duties as assigned.MINIMUM REQUIREMENTSEducation:High School Diploma with required experience, Associate’s Degree in Health Information Management, or Bachelor’s Degree in Health Information Management.
Experience:One year data analysis or a combination of data analysis experience and HIM experience/training.

Knowledge and Skills:Proficient in document validation. Ability to manipulate data from various systems. Strong analytical and problem solving skills. Proficient knowledge of detailed privacy regulations - HIPAA, Corporate Compliance and NJ Department of Health. Proficient in the use of multiple EMR systems.
Special Training:Intermediate proficiency with Microsoft Office including Word, Excel and Outlook. Bi-Lingual preferred.

ESSENTIAL FUNCTIONSProfessionally represents the Capital Health Mission Statement by answering phones promptly and courteously. Answers questions from any caller who needs assistance. Uses various hospital systems to search patient information to help direct callers to the correct department, clinic, or physician's office.Collects, tracks and analyzes information and ensures that information is disseminated and effectively communicated to all levels of the HIM Department including but not limited to the daily DNFB report. Displays results in appropriate format utilizing Microsoft Office Suite.Reviews paper and electronic medical records interpreting and analyzing medical record documentation, and extrapolates pertinent health information.Manages clinical, demographic and health information in multiple systems to support patient care and HIM functions. Troubleshoots and reconciles interface errors.Utilizes multiple EMR applications to analyze and synthesize qualitative and quantitative health information for various and diverse needs, audiences, and regulatory bodies including the DNV.Compiles records from multiple systems in an electronic format to comply with time sensitive record requests from audit contractors.Performs ongoing research to identify the factors influencing industry trends, developments and continuously changing healthcare standards and best practices.Develops health information format and content standards to ensure the collection of complete, accurate, timely, and compliant health information within EMR's. Utilizes automated health information to disseminate to healthcare providers for continuity of care.Grants access to HIM systems based on user role and assures appropriate authorization.Prepares, edits, and proofreads letters, documents, and presentations as requested.Assists in training new HIM staff and copy service staff in the location of medical records in our multiple medical record systems at Capital Health. Provides detailed information to all HIM staff on various locations and types of medical records in Capital Health outpatient departments.Analyzes medical records for documentation deficiencies using several hospital systems where the Capital Health hybrid record resides. Adheres to the Medical Record Documentation Requirements Policy when assigning deficiencies to L.I.P.'s. Is able to keep current to assure records are available to the physician for completion as soon as possible after discharge.Performs data integrity analysis and resolution within several electronic queue's to solve problems when physicians reject a deficiency or are not assigned the correct deficiency. Analyzes and enters data electronically throughout the work day to communicate to other HIM staff and coders when queries have been answered and other parts of the chart have been signed off and are ready for release if requested for release of information purposes.Accurately provides information on delinquent charts to physician's and their offices through electronic means as well as verbally. Coordinates information to the many physician groups at Capital Health by providing a clear picture of all members of their group and the number of charts that are awaiting their completion. This data is utilized for the delinquent chart calculation process and must adhere to the highest accuracy since the penalty is suspension of admitting privileges.Performs concurrent data analysis of deficient charts to assist management in the physician suspension process. Keeps management alerted to any specific difficult situations that physicians are encountering in completing their charts.Performs other duties as needed.PHYSICAL DEMANDS AND WORK ENVIRONMENTFrequent physical demands include:
