Position Summary:

Prepares birth and death certificates, maintains birth and death registers, and maintains statistical information relating to vital statistics.

Facilitates reconciliation of issues surrounding transcription.

Prepares, maintains and disseminates information pertaining to births and deaths as required by the State Bureau of Vital Statistics or other agencies.Obtains obstetrical and pre-natal information from mothers medical record and through contact with Obstetrician as needed. Contacts mothers as needed regarding concern or problems with birth certificates.Prepares and processes death certificates on all hospital deaths.Maintains birth and death registers.Recognizes, and evaluates inconsistencies, discrepancies, and inaccuracies in medical dictation, and seeks clarification to resolve issues necessary to facilitate accurate and complete records.Responsible for answering telephone calls and assisting visitors (including but not limited to physicians, patients, and their family members), while maintaining confidentiality.Performs notary duties, such as paternity affidavits.Assists file clerks with loose report filings.Reports to: Director of Health Information Management
Minimum EducationX High school diploma or equivalent Required or three years of directly related experience.Required SkillsRequires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.Minimum Work ExperiencePrefer previous experience in preparing birth certificates and/or medical records. Two to Three years experience in medical records preferred. Knowledge of Medical Terminology required.
