📝 Job Description:As a Clinical Manager – Integrated Care, you will play an essential role in ensuring smooth and effective transitions from hospital to home, helping to reduce hospital readmissions and improve health outcomes. This role involves direct care, participant education, and coordination with interdisciplinary teams (IDT) to enhance participant outcomes during the post-discharge period. You will also supervise home care staff, ensuring compliance with clinical standards and high-quality care at home.
🔍 Key Responsibilities:Collaborate: Work with hospital discharge planners, physicians, and interdisciplinary teams to develop personalized care transition plans.Review & Educate: Ensure participants understand their post-discharge care plan, including medication management, wound care, and follow-up appointments.Conduct Home Visits: Assess participants' health, living environment, and support systems.Monitor Conditions: Identify changes or complications and adjust care plans as needed.Provide Skilled Nursing Services: Medication administration, wound care, vital sign monitoring, and support with activities of daily living.Educate Participants & Families: Manage chronic conditions, prevent complications, and improve overall health outcomes.Supervise & Mentor: Guide Nurses, Personal Care Aides (PCAs), and Home Health Aides (HHAs) in delivering home health care services.Maintain Documentation: Ensure compliance with PACE, LHCSA regulations, and organizational standards.💼 Essential Skills:Current active and unrestricted license as a Registered Nurse in New York State.Degree from an NLN-accredited nursing school (RN, BSN preferred).At least 3 years of clinical nursing experience, including 1-2 years in home care or a similar healthcare setting.Experience in care transitions, case management, or PACE programs preferred.Strong clinical assessment and critical thinking abilities.Excellent communication skills.Capacity to manage multiple participant cases effectively.🌟 Additional Skills & Qualifications:Experience in geriatrics and managed care.Ability to educate participants and families on health management.Strong leadership and mentoring skills.🏢 Work Environment:This role involves both office work and field visits to participants' homes.Locations include Rockaway, Flushing, Manhattan, and Seneca.The work environment requires adherence to clinical standards and interaction with various healthcare professionals and participants in their homes.Ready to lead and make a difference?
