š Job Description:As anĀ Occupational Therapist, you'll plan and administer therapy treatment programs to restore function, relieve pain, and prevent disability following disease, injury, or loss of body parts. This role operates on aĀ 9:00 AM ā 5:00 PMĀ schedule.
š Key Responsibilities:EvaluateĀ participants referred by the provider and administer treatment programs in collaboration with the provider.CompleteĀ assessments, progress notes, treatment plans, discharge summaries, and interdisciplinary notes.PerformĀ participant evaluations and treatments at the PACE site and in the field (e.g., participantās home, nursing homes).MaintainĀ a clean and safe environment, following safety protocols.ParticipateĀ in developing participant care plans with other interdisciplinary team members.DocumentĀ all pertinent participant information, including evaluations, progress notes, MDS, and care plans.ConsultĀ with physiatrists and other professionals when appropriate.MaintainĀ high standards of health care and clinical practice through continuing education and professional development.ParticipateĀ in the clinical education of student affiliates and provide clinical instruction to COTAs.RecommendĀ equipment for discharges as appropriate.EngageĀ in Performance Improvement Monitoring.ActĀ within the scope of individual authority to practice.MeetĀ standardized competencies established by the PACE organization before working independently.š¼ Essential Skills:Successful completion of an Occupational Therapy program from an accredited university.Bachelorās Degree in Occupational Therapy.New York State licensure as a Registered Occupational Therapist.One (1) year of experience as an Occupational Therapist and/or experience with the Geriatric population preferred.Minimum of one (1) year of experience working with a frail or elderly population, or willingness to receive appropriate training from the PACE organization if less experienced.Ability to establish rapport with patients and staff.Proficiency in using all OT modalities and equipment.Ability to travel to participants' homes, nursing homes, and various sites/clinics.š Additional Skills & Qualifications:Knowledge of PACE regulations.Familiarity with Care Compass and other EMRs preferred.š¢ Work Environment:The work environment includes both the PACE site and field locations such as participantsā homes and nursing homes.The position requires traveling to various locations for participant evaluations and treatments.The Occupational Therapist will work in a collaborative and interdisciplinary team setting, adhering to safety protocols and maintaining a clean and safe environment.Professional attire is expected while on duty.Ready to make a difference?Ā Apply now and join our team in delivering exceptional care and support to our community!
