**UPMC Home Health is hiring a Home Care Aide (Harrisburg, Carlisle area)** Provide designated care under the direction of the primary service provider in the home, in order to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home. casual, no weekend requirement **Responsibilities:** + Must maintain annual competency evaluation scores as determined by regulatory requirements + Attends compliance training and adheres to the organization standards of conduct, policies and procedures. + Proven ability to facilitate change. + Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction. + Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide. + Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization. + Participates in all required agency meetings. + Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient?s condition to appropriate professional staff. + Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards. + Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional. + High School Diploma or GED - Required + One (1) year experience - Preferred + Certified Nurse Aide- Preferred + Minimum of 75 hours of Nurse Aide training Required. + If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire. **Licensure, Certifications, and Clearances:** + Automotive Insurance + Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) + Driver's License + Act 33 + Act 34 + OAPSA **UPMC is an Equal Opportunity Employer/Disability/Veteran**
