You will work Monday through Friday on daylight shifts from 8:00 a.m. to 4:30 p.m. (Weekends on a rotating schedule). The role of the home health aide is to provide patient-centered care to persons with life-limiting illnesses. The home health aide will aid with bathing, dressing, and other activities of daily living. The HHA will provide education and demonstrate care to caregivers to support the goal of remaining at home for the end of life. Quality care is patient-centered and the HHA goal is to ensure all patients are treated with respect, dignity, and compassion. There is some flexibility to scheduling. One-on-one care and relationship building with peers and patients. Provide designated care under the direction of the primary service provider in the home to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home.
