**Home Care Educator**
**Summary:** The Home Care Educator assesses, develops, implements, and evaluates educational programs for home care and hospice staff, ensuring compliance with regulations. Collaborates with managers for orientation, competency development, and ongoing staff development. Acts as an informatics liaison, providing technology education and coordinating student learning activities.
**Essential Functions:**
**Orientation & Integration:**
+ Develops and implements individualized orientation schedules and learning plans.
+ Supervises field orientation and provides ongoing feedback.
+ Ensures orientation materials are current and relevant.
+ Acts as a coach and mentor to new colleagues.
**Educational Needs Assessment & Development:**
+ Develops and implements education to ensure regulatory compliance.
+ Coordinates monthly and ongoing education for Home Health Aides.
+ Assesses colleague competency and develops effective training strategies.
+ Maintains education records and ensures survey readiness.
+ Provides clinical education and observes clinical practice as needed.
+ Utilizes diverse teaching strategies and adult learning principles.
**Technology & Documentation:**
+ Educates and supports clinicians with electronic health records (HCHB).
+ Maintains current understanding of clinician-based technology.
**Clinical Learning Coordination:**
+ Coordinates student placements and supervises learning activities.
+ Ensures compliance with regulatory requirements for student affiliations.
**Patient & Community Education:**
+ Develops educational materials for patients and the community.
**Program Development & Participation:**
+ Contributes to policy development, competencies, and continuing education activities.
+ Serves on committees and participates in special projects.
**Qualifications:**
+ Graduate of an approved Nursing education program (bachelor’s preferred).
+ Masters in OT or ST or Doctorate in PT considered (Home Care only).
+ Certification in OASIS within one year (Home Care only).
+ Ten years of nursing practice, with five in Hospice preferred.
+ Certification in Hospice and Palliative Care within one year (Hospice only).
+ CPR Instructor Certification preferred.
+ Proficient in Microsoft applications.
+ Strong oral and written communication skills.
