The Home Care Coordinator will schedule and coordinate all services rendered and carry out delegated office duties. Responsibilities also include:
+ Supports agency operations by maintaining office systems and workflow.
+ Creates and maintains staff schedules. Enters scheduling data. Assists in maintaining clinical records.
+ Conducts intake interviews and communicates with Nurse Supervisor about potential clients. Records client admissions, client transitions, discharges, etc. Contact care providers and clients regarding schedule and day-to-day changes.
+ Maintains call-in/call-out records and activity sheets. Prepares billing and payroll. Performs general office duties including but not limited to reception, taking and directing calls, sorting and distributing mail, etc.
+ Assists in recruiting and orientating CHHA's. Works with HR Departments regarding HR policy compliance. Maintains personnel records.
+ Assists in marketing and planning events, including attendance at marketing functions.
Requirements:
+ High School Diploma A current
+ 1-2 years' experience in Home Care Coordination and Scheduling.
+ Experience in office procedures for Health Care Services Firm.
