The salary range for this position is $36.00 - $59.65 (Hourly Rate)

Placement within the salary range is dependent on several factors such as relevant work experience and internal equity. For positions represented by a labor union, placement within the salary range is guided by the rules outlined in the collective bargaining agreement.

Description

The Home Care Coordinator reflects the mission, vision, and values of Northwestern Memorial HealthCare (NMHC), adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

To coordinate services between Palos Health and the PH Home Care for patients who have chosen Palos Health Home Health Care services.

To function as Home Care resource to other hospital departments and members of the community.

Responsibilities:

Attends interdisciplinary rounds at Palos Health as requested to facilitate the coordination of services from hospital to home.Consults with physicians, nurses, social service and other members of the Health Care Team at Palos Health to determine patient needs and establishes a coordinated program of care, products and services prior to discharge from any unit or departmentFacilitates discharge planning of current PH Home Care patients or are hospitalized at Palos Community Hospital.Identifies hospitalized patients of Palos Health Home Health Care who may require initiation of services during the evening or weekend hours.Utilizes community agencies and Palos Health Social Services Department in a consistent manner to develop home care plan post-discharge.Educates patients and families regarding available resources post hospitalization.Assesses patient and family support system to determine appropriateness of Home Care to meet post discharge needs.Receives physician orders for Home Care services and communicates relevant clinical, psychosocial, and financial information to Home Care supervisors.Documents all communication with other members of the health care team including, but not limited to, the patient, family, physician, social service, and discharge planners.Participates in staff meetings, inservices, management meetings, process improvement, and other activities and meetings as requested.Generates monthly tracking reports as requested.Assists nursing supervisors with verification of benefits as necessary. Informs patients of possible financial responsibilities whenever possible.Educates Social Workers, Physicians, Nurses and other members of the health team regarding Federal, State, Local, and regulatory/accrediting rules and regulations governing home care.Educates Nurses, Physicians, Social Workers, and other members of the Health Care Team, regarding agency policies and procedures and products/services available in the home care industry.Demonstrates ability to care for patients with understanding of the cognitive, physical, emotional, and chronological maturation process.Adheres to PH Home Health Care policies regarding all aspects of HIPAA Privacy and Confidentiality Rules, dress code and attendance.Adheres to all PH and PH Home Health Care policies.Performs other and related duties as directed.

Qualifications

Required

Current licensure as a Registered Nurse in the State of Illinois.Minimum 2 years of previous home health experience.     

Preferred

BSN preferred.Previous home health management experience preferred.
