We are currently looking for a Registered Nurse to join our team as a Home Care Intake Coordinator. The position would be responsible for providing assessment of patients’ home health care needs and coordination of referrals to home care and hospice. The person in this role will be stationed at one of our partner hospitals such as; Clara Maass Medical Center or one of the other major hospitals we partner with in Essex County.

Responsibilities Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workersCoordinates referrals to home care to promote continuity of careProvides individualized counseling to patient and family regarding available home care servicesEstablishes and maintains professional working relationship with hospital staff, physicians, and VNAHG staffProvides back up coverage to assure availability of intake coordinator servicesAdheres to the organization’s policy in regards to absenteeism and appearanceOmission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position Qualifications RN; Bachelor’s degree in Nursing preferredValid NJ nursing license2+ years of community health nursing experienceExcellent verbal and written communication skillsExcellent interpersonal and organizational skillsStrong client focus, assertive, decision making skills, articulate, and creative in problem solvingStrong decision making skills 

Working Conditions/Physical Demand: Fast paced health care environment with phone, scanner, email, fine motor hand movement and computer use. Use of personal automobile with valid insurance and driver’s license
