Sentara Home Care has an opening for a Social Worker MSW. This part time position provides social work services to home care patients.

Shift: Monday - Friday (Rotating)

The MSW performs a variety of casework and social work services to patients and families. Must develop and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.

Qualifications:

Master of Social Work requiredSocial Work experience required
