Eddy Visiting Nurse & Rehab Association has an exciting opportunity for an Occupational Therapist responsible to manage a caseload of patients requiring home-based therapy- primarily post acute or subacute- in their homes.

**What you will do:**
+ Promote/provide Occupational Therapy services in the home to assist the patient in achieving his/her optimal level of functioning.
+ Evaluate and treat individuals with functional deficits secondary to neurologic, orthopedic and medical conditions.
+ Teach patients/caregivers and help them to become independent with their treatments and self-management of their chronic illness(es)
+ Coordinate an interdisciplinary plan of care with nursing/physical/occupational/speech therapists, medical social workers, aides and dieticians.
+ Working hours are generally 8am to 4pm, some flexibility in schedule. Looking ideally for 3 shifts (8a-4p) per week (M-F)
+ Minimal weekend and holiday rotations
**What you will need:**
+ NYS Occupational Therapist Licensure
+ 1 year Occupational Therapy experience is required
+ Home care experience preferred but not required
+ Valid Driver's License and Reliable Vehicle. This position provides mileage reimbursement.

**Pay Range:** $35.85 - $45.50 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
