**Employment Type:** Full time **Shift:** **Description:** **Position: Home Care Physical Therapist** **Location:** Hartford Region **Employment Type:** Flexible Scheduling Opportunities- Part Time **Benefits Start:** Day 1 **Bonus** : $5,000 **Position Purpose:** As a **Home Care Physical Therapist** with Trinity Health Of New England At Home, you will provide professional physical therapy services to clients as prescribed by their physician. This role requires compliance with the state's professional practice act for physical therapy, any applicable licensure and certification requirements, and the organization’s policies and procedures. **Key Responsibilities:** + **Client Assessment & Planning:** Accurately assess clients at admission and other required time points using standardized documentation. Collaborate with the physician and client to develop an assessment-based treatment plan with attainable goals to improve or stabilize the client's condition and promote independence. + **Visit Prioritization:** Effectively prioritize and plan visits based on client capabilities, condition, physician orders, and available resources, in collaboration with the Team Leader or designee. + **Regulatory Compliance:** Maintain a working knowledge of conditions of participation and applicable federal, state, and local laws and regulations, including the Organizational Integrity Program, Code of Ethics, and agency policies. Adhere to these standards with honesty, ethics, and professionalism. + **Care Coordination:** Make appropriate referrals to other disciplines and services, and coordinate care with others to ensure effective and efficient service delivery. + **Therapeutic Interventions:** Utilize professional judgment to assess clients and formulate therapy interventions. Initiate and implement appropriate preventive, therapeutic, and rehabilitative procedures and techniques. + **Communication & Reporting:** Report changes in client condition promptly to the client’s physician and obtain orders for any changes in the treatment plan.
