Home Care Scheduler Responsibilities:

The Team Assistant is responsible for scheduling and oversight for all disciplines: Nursing, Therapy, and Social WorkAssists in entering approved PTO and patient Emergency Management Plans into scheduling packageEnters and verifies vital patient information into billing systemMaintains CPR card, Driver's license and Auto Insurance information in team member filesReviews admission packets for completenessAssists the Home Care Supervisor in productivity oversightWorks all report as indicated in standard workAdds and approves all disciplines as instructed by the Supervisor, including the visit typeEnters skills for each clinician in staff management area of scheduling package and enters assignment of and updates for case manager for each patientAssists with daily routine paperwork and clerical duties necessary for the smooth day to day function of the unit

Education:

Required High School or EquivalentPreferred Associates Computer Related

Experience:

Required 1 year Computer Related Preferred 1 year Home Care
