We are seeking a dedicated and compassionate Home Health Registered Nurse (RN) to join our team in San Ramon. This role involves providing high-quality, patient-centered care to clients in their homes, ensuring their comfort, safety, and well-being.

Key Responsibilities:

Conduct comprehensive assessments of patients' health conditions and develop individualized care plans. Administer medications, treatments, and procedures as prescribed by healthcare providers. Monitor patients’ progress and adjust care plans as needed to achieve desired outcomes. Educate patients and their families about disease management, treatment plans, and home care techniques. Collaborate with physicians, therapists, and other healthcare professionals to coordinate care. Maintain accurate and timely documentation of patient care and progress.

Qualifications:

Current RN license in the state of California. Minimum of 1 year of nursing experience, preferably in home health or a related field. Strong clinical skills and the ability to work independently. Excellent communication and interpersonal skills. Valid driver's license and reliable transportation.
