**Responsibilities**
The Home Health/Hospice Aide provides personal care, homemaking services and other supportive services for people in their place of residence. All such services will be provided in accordance with an individualized, comprehensive patient care plan and under the supervision of a Registered Nurse and/or a Physical, Occupational, or Speech Therapist, when appropriate, in compliance with the agency’s policies and standards of practice.
**Qualifications**
18 years or older and completion of a state approved license program for Home Health Aide or Nursing Assistant training program required. If working in the state of Wisconsin, must be registered on the state Wisconsin registry.
**Additional qualifications:**
Previous work experience as a certified nursing assistant within the past 12 months is preferred. Experience with end-of-life or geriatric populations preferred. Must possess clear written and verbal communication and excellent interpersonal skills; ability to read, write and carry out directions. Must be able to work independently. Must foster team atmosphere by maintaining open communication with interdisciplinary team; must have the ability to recognize problems/concerns that should be brought to the attention of the proper personnel. Basic computer and technological device skills. Ability to assemble, maintain and record data in an accurate record. Able and willing to drive-in all-weather conditions and cover service area. Ability to work 24 hours a day, 365 days a year. Must provide personal mode of transportation to patient’s place of residence.
**License and certification:**
Valid driver’s license, proof of auto insurance, and acceptable driving record as defined by policy required. Maintains current Basic Life Support for Health Care Providers from one of the following programs: American Heart Association or American Red Cross. If a candidate does not currently have their Basic Life Support for Health Care Providers from one of the following programs: American Heart Association or American Red Cross the Basic Life Support, certification will be included as part of the orientation program. If working in the state of Wisconsin must be a registered certified nursing assistant on the State of Wisconsin Registry.
