Position Summary

Accurate Home Care has an opening for a Home Health Clinical Supervisor who will be responsible for the oversight, coordination, and supervision of skilled and non-skilled home health services in patient homes. Requires timely and appropriate care interventions and interdisciplinary communication to enhance patient satisfaction.

What will I be doing?

Responsible for oversight of all patient care services and personnel, including,
but not limited to:

Making patient and personnel assignments. Coordinating patient care. Coordinating referrals. Ensuring that patient needs are continually assessed. Ensuring the development, implementation and updates of the individualized plan of care. Plans, coordinates, delivers, and oversees the delivery of outcomes of required healthcare related services/training programs on a routine basis; monitors both skilled and non-skilled healthcare services. Provides clinical teaching and conducts performance monitoring, identifying and reducing risk. Facilitates communication and serves as a liaison between and among nursing and direct care staff, family and/or guardians, and community medical providers and professional consultants in cooperation with operations management. Oversees start of care, assessment, and first home visit of all home health patients. Directly supervises Nurse Case Managers in the implementation, coordination, maintenance, evaluation, and termination of the individualized plan of care, analysis outcomes, medical records, and physician orders. Demonstrate critical thinking to advocate for patient safety, adherence to company policies, and conflict resolution. Understand and comply with policy and procedures set forth by AHC. Monitors operations to ensure compliance with county, state and federal regulations affecting home health care. Assists in analysis and preparation of operating budget for service line. Quality Assurance and Performance Improvement analysis, including quality validation of visit documentation completed by Nurse Case Managers. Additional tasks, as assigned by manager.
