Job Description:

Serves as a member of the health care team who works under the supervision of the Registered Nurse. The Home Health/Hospice Aide is an integral member of the team while in the field carrying out the patient's plan of care assigned by the RN. They are responsible to provide assistance to our patients and families so they may maximize their independence and autonomy while remaining in their home (private residence, nursing home, assisted living).

Major Responsibilities:
1. Assists with basic care for patients in the areas of nutrition, elimination, comfort, mobility and safety in patient homes.
2. Provides more individualized care as delegated by the RN though reinforcement of education providing dressing changes, and by providing household services essential to health care at home (ex. dishes, laundry, simple meal prep).
3. Maintains visit schedules in the patient's home as identified by the multidisciplinary health care team and the patients and families.
4. Implements the patient and family care plan under the indirect supervision of the RN.
5. Works collaboratively with the multidisciplinary team members to effectively communicate and document accurate information.
6. Assists patients with self-administration of medications as delegated by the RN and as evidenced by on-site supervision/care provision, client satisfaction or documentation.
7. Runs and reviews reports to maintain compliance measures as well as inputs demographic, clinical information and data into the clinical software system.
8. Demonstrates dependability, organizational skills and effective use of time by setting priorities to complete delegated work within established time frames.
9. Demonstrates role accountability by adhering to Gundersen Health System’s Mission statement, policies and confidentiality standards.
10. Assists in the orientation of new personnel.
11. Participates in quality improvement activities, contributes ideas and demonstrates a commitment to make improvements in the quality of services provided.

12. Adheres to regular and predictable attendance.

13. Personally interacts, engages with and cares for patients.

14. Ensures patient safety.

15. Performs other duties as assigned.

Education and Learning:
REQUIRED
High School Diploma or equivalency + completion of a Home Health Aide program that meets state and federal requirements for placement on the State Home Health Aide Registry
 

Work Experience:
REQUIRED
1 year experience in acute care, nursing home or home health agency.

DESIRED
2 years experience in acute care, nursing home or home health agency.


License and Certifications:
REQUIRED
Current Infant-Adult Basic Life Support for Health Care Providers (with hands on learning) from one of the following programs; American Heart Association or American Red Cross.
