Required Qualifications:

Graduate of an accredited Licensed Practical/Vocational Nurse program Upon hire: Washington Licensed Practical Nurse Within 30 days of hire: National Provider BLS - American Heart Association Within 30 days of hire: National Provider Identifier (NPI) number Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.
