Responsibilities

No on-call requirement/Flexible schedules.  Provides care and education to patients and their caregiversParticipates in the coordination of services with other disciplines and community resources as neededEvaluates and revises care plans/assessments based on changes in patients and/or their environmentsParticipates in regular case conferences and in-services or special case orientations as needed and participates in on-call rotation
Qualifications

Active LPN LicenseOne year of clinical experience as a LPN requiredValid Driver’s LicenseIntuitive knowledge of technology and comfortable with Smartphones, tablets, virtual reality, interactive software and artificial intelligenceIn Washington State, all providers must meet NPI requirement and furnish proof prior to onboarding
