Position Overview:
We are seeking a dedicated and compassionate Per Diem Home Health Nurse (RN/LPN) to join our team. This role involves providing nursing shifts in clients' homes, attending to all their needs, including personal care and skilled nursing services. Enjoy the flexibility of per diem work, with the autonomy to accept or decline shifts based on your availability and preferences.

Responsibilities:
- Provide comprehensive nursing care to clients in their homes.
- Assist with personal care tasks, including bathing, grooming, and dressing.
- Perform skilled nursing tasks such as wound care, medication administration, and monitoring vital signs.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Conduct thorough assessments to monitor clients' health status and identify any changes or needs.
- Educate clients and their families on disease management, prevention, and healthy living practices.
- Maintain accurate and timely documentation of all client interactions and care provided.
- Communicate effectively with clients, families, and other members of the healthcare team.

Qualifications:
- Current RN license in the state of Florida.
- Minimum of 1 year of nursing experience, preferably in home health or a related field.
- Strong clinical skills in personal care and skilled nursing tasks.
- Excellent assessment and critical thinking abilities.
- Compassionate, patient-centered approach to care.
- Strong organizational and communication skills.
- Ability to work independently and manage time effectively.
- Reliable transportation and a valid driver's license.
