The Home Health Physical Therapist is a clinically competent, licensed, professional of his/her discipline who assumes responsibility and accountability for a group of Home Care/Home Health residents/patients through the process of evaluation, ongoing assessment, planning, and implementation of therapy interventions. Effectively and professionally interacts with residents/patients, family members, responsible party and other health care team members, while maintaining standards of professional conduct. This position must comply with regulatory standards of the American Association of Physical Therapy, the Department of Health, and any other professional guidelines or standards.
Responsibilities Perform initial assessment on residents/patients and re-assessments as per MD order and within department policy.Develop and revise plan of care as indicated by resident diagnosis, physical and cognitive status, functional goals/outcomes, and discharge plan.Provide treatments to meet individual’s needs (physical, cultural, and psychosocial) per plan of care.Plan for and provide resident, family and caregiver education, identifies learning needs, clearly communicates information, and documents response to education provided.Document and complete billing according to current standards and policies.Plan for resident discharge, anticipates long term and short term resident needs, collaborate with interdisciplinary team, and assesses resident prior to discharge as needed.Delegate and collaborate in providing resident care according to licensure and skill levels, follows up to ensure success.Attend meetings, workshops, seminars and participates in committees as requested/required by facility.Promotes the Benedictine Mission and Core Values of Hospitality, Stewardship, Respect and Justice by bringing the Mission and Core Values into the day-to-day activities of the company. Current state licensure in Physical Therapy, which must be in good standingBachelor’s Degree
