BayCare is currently in search of our newest Per Visit Home Health, Physical Therapist Assistant who is passionate about providing outstanding customer service to our community. We are looking for an individual seeking a career opportunity with one of the largest employers within the Tampa Bay area.

Home visits in assigned service area: Manatee County, FL

The Per Visit Home Health Physical Therapist Assistant:

Assists Physical Therapist with the administration of interventions utilized in physical therapy and rehabilitationMay progress patients in the treatment plan developed by the physical therapistCommunicates with all physicians involved in the plan of care and other healthcare practitioners (as appropriate) related to the current plan of careFollows established procedures, emergency and safety precautions at all times in the treatment of patients and use of modalitiesMaintains timely communications, records, reports, and other documentation in accordance with agency policies & procedures, and Medicare guidelinesCaptures documentation during the home visit following company policy using company issued laptopAssists in progressing therapy programs with prior assessments and approval, prepares clinical progress notes and participates case conference and utilization reviews

Required Certifications and Licensures:

Physical Therapist Assistant State of Florida LicensureDriver's License State of Florida (Florida driver's license is required within 30 days of hire)Basic Life Support

Education:

Associates Degree Physical Therapy Assistant

Experience:

1 year Physical Therapy Assistant experience

Preferred 1 year Home Care
