We are seeking a compassionate and skilled Registered Nurse or Licensed Vocational Nurse for our Home Health. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their health and well-being. This role requires a strong understanding of home care practices, senior care. The Nurse will collaborate with healthcare teams to develop and implement patient care plans tailored to individual needs.
Duties
Conduct comprehensive assessments of patients' health status and needs.
Develop and implement personalized care plans in collaboration with patients, families, and healthcare teams.
Monitor patients' progress and adjust care plans as necessary.
Provide education and support to patients and families regarding health management and home care practices.
Ensure compliance with healthcare standards and regulations in all aspects of patient care.
Maintain accurate records of patient care activities, including assessments, interventions, and outcomes.
Collaborate with interdisciplinary teams to coordinate comprehensive care for patients.
Requirements
Current Registered Nurse (RN) license or LVN License in the state of practice.
Must have two years experience as a nurse.
RN is required to complete Outcome and Assessment Information Set (OASIS).
LVN is required to complete weekly visit notes that meet home health requirements.
Strong communication skills to effectively interact with patients, families, and healthcare professionals.
Ability to visits patients in the Los Angeles area.
Current CPR, TB, Health clearance and clinical skills self assessment.
