WesleyLife Home Health is celebrating 20 years of providing exceptional home health services throughout central Iowa. We serve clients in Boone, Dallas, Hamilton, Jasper, Marion, Marshall, Polk, Story, Warren, Cass, Washington, Audubon, Guthrie, Adair, and Poweshiek counties, offering personalized, focused, one-on-one care wherever our clients call home.
We’re restructuring our Home Health department to let nurses focus on the work they love. Whether your passion is client care, case management, leadership, or admissions, we have a role for you. Current Openings
Nurse Case Manager-RN
This position offers a hybrid local remote work arrangement, allowing for in-office and remote work flexibility. However, candidates should reside in one of the 15 counties we serve in central Iowa to ensure availability for onsite work, training and mentoring new team members, meetings, and performing admissions, client visits, and supervisory visits. Local remote work will be available after the initial training period is completed. Provide front-line supervision for a team of nurses, physical or occupational therapists, and home care aides. Train and mentor new and existing team members. Partner with the Clinical Manager on constructive feedback and performance reviews for nurses. Manage cases, coordinate care, and initiate discharge planning. Perform admissions, client visits, and supervisory visits.Admissions RN
Travel throughout our service area to handle admissions, discharges, resumptions of care, and recertification assessments. Conduct other client visits as needed.Visit Nurse-RN
Perform admissions, recertifications, resumptions of care, discharge assessments, and routine client visits. Counsel clients and families about nursing and related needs. Communicate with the Nurse Case Manager about healthcare provider orders, necessary equipment, incident reports, and changes in client condition.Compensation
The pay range for these positions is $65,375 - $88,240 / year. The starting pay rate will be based on which position you’re applying for and your years of experience.What You’ll Gain

You’ll also need a valid, unrestricted driver’s license, a reliable vehicle, and current insurance on that vehicle that isn’t SR-22.
