Clinical activities will include patient education, therapy management, clinical monitoring, nurse education and direct communications with patient/caregiver, nurse and provider. Position requires participation in the Quality Assurance and Quality Improvement Plan. Qualifications EDUCATION, EXPERIENCE, TRAINING Required qualifications: 1. Active pharmacist license in the state of Illinois2. Bachelor’s degree in Pharmacy or Doctor of Pharmacy (PharmD)3. At least 1 year of experience in hospital or home infusion pharmacy4. At least 1 year of experience in sterile compounding5. Critical thinking, decisive judgment, and ability to work independently are required.6. Excellent communication, organizational, and customer service skills are required. Preferred qualification: 1. Knowledge in 340B program operations and compliance2. Proficiency in Caretend electronic medical records (EMR) system3. Knowledge of outpatient pharmacy revenue cycle as it relates to prescription and medical benefits Ressurection Medical Center offers competitive compensation and a reasonable compensation estimate for this role is $72.33 to $72.33.
