Key Position Details:

Job Description:

Responsible for acceptance and evaluation of referrals for Allina Home Care Services programs including Hospice, Palliative Care and Home Health within Allina facilities and any other community based facilities as assigned. Facilitates patient transitions from acute care, non-Allina or other community based facilities to the most clinically appropriate Home Care Services. Liaison will assess and assist patient , family and other caregivers to identify any/all patient post-acute services needs that can be provided by Allina Home Care Services or other community based services as warranted.

Principle Responsibilities

Care CoordinationVisits acute care patient at bedside upon referral to explain referred Home Care services.Completes referral documentation including admission forms.Communicates with patients/caregivers/physicians for informed decision-making to access most appropriate post-acute care.Advises regarding insured/Medicare/Medicaid or private insurance coverage availability.Provider CoordinationCoordinates with clinical programs and facilities to facilitate smooth admission processes.Ensures physician orders, labs, and other patient documentation to validate referral and initiate service is obtained.Facilitates communications with referring facilities, primary or attending physicians and clinical program staff.Communication and EducationProvides in-service education and information to physicians and community facilities regarding Home Care Services.Participates in in-service, skill and competency development, and other appropriate education to maintain clinical knowledge and skills.Quality and Process ImprovementParticipates in process improvement efforts to achieve the most efficient and effective patient care transitions.Assists in data collection and quality, safety, and customer service improvement initiatives.Other duties as assigned.


Required Qualifications

2 to 5 years in Care Management, utilization, care coordination or discharge planning


Preferred Qualifications

Bachelor's degree (RN/PT/OT/SLP/LSW accepted) with valid clinical license for discipline


Physical Demands

Light Work:Lifting weight Up to 20 lbs. occasionally, Up to 10 lbs. frequently

Pay Range

Pay Range: $39.19 to $53.72 per hour

The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
