Essential Functions of the position are detailed below, and include any physical requirements below: 1.Provide family-centered, home-based support services (weekly, biweekly or monthly) to pregnant women and families with children (up to age five). 2.The goal is to enhance parenting skills, promote healthy pregnancies, and foster infant/child development while supporting prenatal and postnatal care. 3.Conduct home visits weekly, biweekly or monthly, tailored to the needs of families with children aged 0 to 5 4.Deliver family-centered services to enhance parenting practices, support healthy pregnancies, and promote child development. 5.Participate in community engagement activities to raise awareness and recruit new families into the program. 6.Represent the program at community events and collaborate with local organizations. 7.Coordinate with program staff, healthcare providers, and community partners to ensure comprehensive family support 8.Assist families in accessing appropriate services and referrals for additional support when needed. 9.Track and monitor family progress, offering guidance and problem-solving strategies. 10.Utilize a program-based curriculum to promote the achievement of family goals. 11.Maintain detailed, accurate, confidential reporting of family interactions, assessments, and referral records 12.Prepare and submit timely reports on family progress, service delivery, and outcomes per program guidelines. PLEASE NOTE: Essential functions include all other duties and responsibilities as assigned:X Lift up to 25 poundsX Climb up and down up to 4 flights of stairs at a timeX Push and pull objects up to 25 pounds Comply with program and/or Agency requirements related to: X Background check, including any program specific requirementsX Driver's License and reliable transportation Immediate Supervisor: Program ManagerDirectly Supervises: N/A Education and Experience Requirements: Relevant Education: Preferred: Bachelors-Community Health, Early Childhood Development, Social Work, Counseling, Psychology or Education Minimum: Associate-Community Health, Early Childhood Development, Social Work, Counseling, Psychology or Education Relevant Experience: Preferred: 4-6 Years Minimum: 2-4 Years Certification/Licensure: Preferred: CPR and First Aid Minimum: N/A
