Overview To provide skilled nursing interventions in the treatment of the patient illness, rehabilitative needs and preventative care. Utilizes the holistic approach in the provision of problem specific therapeutic interventions, teaching and training activities in accordance with the plan of care. Problem specific interventions oriented to achieving outcomes are reflected in his/her ongoing documentation. Responsible to apply professional knowledge and skills in accordance with accepted standards of clinical practice, to facilitate problem resolutions and the achievement of individualized patient goals and outcomes. Practices under the direction and supervision of the registered nurse to proactively meet the needs of and assist the patient in the achievement of individualized, defined and measurable outcomes. Must successfully pass an AHCA Level 2 screening for this position Qualifications Minimum Education and Experience Requirements: Licensed as a Practical Nurse in the State of Florida. One year of clinical experience as an LPN preferred. One year of experience in the specialty of home health is preferred. Motor Vehicle Operator Designation: Employees in this position: Will operate vehicles for an assigned business purpose as a 'frequent driver' NOTE: A frequent driver is defined as one who uses his/her personal or Shands automobile a) at least once daily, b) at least five individual trips per week or c) drives, on average, over 150 miles per week in the performance of his/her job.
