POSITION TITLE:         HOPE Village Case Manager

STATUS:                          Regular-Full Time/Non-exempt

EDUCATION AND WORK EXPERIENCE:

• BA Degree in Social Work preferred.
• 2 years of demonstrated experience in case management and or related social services.
• Demonstrates strong understanding of: Housing First, Coordinated Entry (CEA), Rapid Rehousing (RRH), City, County, and Partner Agency Human services
• Strong Human relations skills; trauma-informed care; community building, and motivational interviewing.
• Competent in HMIS entries, exits, and reporting.

• Complete safety orientation and comply with L/I requirements regarding chemicals and hazardous waste
• Obtain documentation of current TB test.
• Complete the Bloodborne Pathogen training within 90 days of employment.

SOFTWARE-RELATED SKILLS:

• Expertise in Outlook, Microsoft Word, and Excel required
• Demonstrates expertise in web resources and navigation
• Working knowledge of integrated database applications and/or ability to use new software programs with basic training
• Competent in HMIS entries, exits, and reporting

SCOPE OF POSITION:

The HOPE Village Case Manager is a client-centered advocate with a primary objective of aiding individuals in overcoming barriers to stability and housing readiness. The Case Manager strives to provide and resource the client with the assistance needed to address each household’s immediate housing crisis. The Case Manager is an expert in identifying and connecting clients to other appropriate mainstream resources and services in the community that aid in stabilization and lead to housing readiness.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• In coordination with the site manager, the case manager will perform the intake of program participants and establish a service delivery timeline.
• Create and update Individual Service Plans informed through weekly strengths-based case management meetings.
• Coordinate with Housing Navigator to refer participants ready to engage in housing search services.
• Work with other agencies/case managers to provide proper support around barriers to housing based on assessments and case management.
• Maintain weekly contact with clients while they are working on their housing assessment and housing readiness plan.
• Provide expert case management for clients with varying degrees of vocational, social, cultural, and/or medical impairments hindering self-sufficiency.
• Provide formal and informal referral services to community resources.
• Coordinates services with other agencies as needed for behavioral health support, employment security, substance abuse programs, vocational training
• Participates in unit staff meetings and in applicable training as assigned;
• Maintains client case records, Well-Sky social services system, HMIS, and required forms and reports are maintained in the prescribed format.
• Performs other administrative and operational functions as requested

PHYSICAL REQUIREMENTS:

• Ability to sit, walk, stand, bend, squat, climb, kneel, and twist on an intermittent or continuous basis
• Ability to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead
• Ability to operate telephone
• Ability to operate a desktop or laptop computer
• Ability to lift up to 45 lbs (usually file boxes)
• Ability to understand written information

Driver Requirements:

Must have a valid WA state Driver's License. Must be able to pass WA MVR screening . Must be able to drive own or TSA vehicle( Based on availability)  Must be able to pass in-house driver training.

Health Insurance: Low bi-weekly premiums ($34.62) for employee only coverage, Kaiser HMO for those residing in a Kaiser service area, Anthem EPO/PPO is available for those residing outside of a Kaiser service area.  Voluntary +1 and +family coverage at additional cost.  Delta Dental DHMO and DPPO dental insurance offered starting at $2.31 bi-weekly cost.  Employees are covered by an employer paid life insurance policy.  Voluntary supplemental life, short-term and long-term disability plans are available.

Retirement Plans: Employer-funded Money Purchase Pension Plan (Defined Contribution Plan) 50% vested at 5 years eligible service time, employee funded voluntary 403(b) options

Parental Leave: Benefit will be one week (5 business days) of paid leave at the employee’s normal rate of pay for births, adoptions and foster placements.

Sick Leave: 12 days annually accruing from day one, eligible for use after 3 months’ service time. 

Paid Vacation: 2 weeks annually accruing from day one, for non-exempt positions. 4 weeks annually accruing from day one, for exempt positions. Accrued vacation eligible for use after 6 months’ service time.

Paid Holidays: 13 designated holidays + 1 floating holiday per year


SUPERVISOR: HOPE Village Site Manager

Working Conditions:  Work is primarily conducted in a sheltered environment, with occasional outdoor duties. The indoor setting ensures a consistent and predictable work environment, free from the discomfort of inclement weather changes. However, some discomfort may be related to noise, dust, dirt, and similar factors. For the occasional outdoor tasks, please dress according to the weather forecast.

Work Schedule / Location:  This position is located at our shelter in Longview, Washington. This a full-time role, and the schedule will be provided on the first day of employment.
