As part of a person-centered care model, this role is responsible for the supervision of hospice care aide services: · Maintains compliance with regulatory agencies as evidenced by the ability to apply Agency policies and procedures resulting in deficiency free surveys. · Provides support and guidance to care management teams concerning performance problem resolution. · Resolves patient care problems collaboratively with internal/external customers. · Anticipates customer needs and responds quickly, accurately and pleasantly. · Implements and follows through on hospice aide care new hire orientation program with input from Care Management and Quality Management departments. · Coordinates hospice care aide new hire orientation process including competency validation. · Participates in the development of annual in-service programs in association with the Care Management and Quality Management departments. · Monitors and communicates regulatory and accreditation changes relative to the hospice care aide area. Functions as a resource to others. · Ensures appropriate resource utilization to meet client needs. · Manages hospice care aide pool, providing guidance to scheduling function. · Hires, trains, coaches, and evaluates hospice aide staff. · Conducts home visits with staff to supervise and evaluate performance; identifies and follows through with development plan. · Determines staffing needs and coordinates hiring to meet service demands. · Participates in preceptor programs and student education. · Will actively participate in performance improvement processes. Minimum Requirements Education High school diploma or GED Experience Two or more years in home care Preferred Requirements Education Home health aide training program or CNA Two or more years in hospice care, preferably in a leadership capacity We take great care of careers With locations around the state, Hartford HealthCare offers exciting opportunities for career development and growth.
