$55hr  + $.65 cents/mile 

Pay = $2530 TOTAL weekly pay for travelers = $1200 taxable + $1330 stipends 

Hospice – Case Managers

Previous hospice/case manager experience preferred 

Must have: EPIC Experience 

Will be driving to patient homes and facilities – DO NOT work in the main Agrace building. Must have an active drivers license. 

Coverage area: Baraboo / Sauk / Merrimac / Lodi  OR  Waunakee / Dane 

Days Shift : 8am-5pm 

No Call

No Weekends
