Hospice Chaplain
We are hiring a Chaplain/Bereavement Coordinator to join our dynamic care team with Hospice of Western & Central Massachusetts in the Worcester area! 24 hours per week with the possibility of growing to full-time. Weekly pay! This is an on-the-road position and the candidate must have a reliable and insured vehicle. The Chaplain/Bereavement Coordinator plans, implements, and maintains the bereavement program and spiritual care services for our residents and their families. Education & Experience: + Bachelor’s degree or equivalent theological degree from college or university. + Graduate of an accredited seminary or school of theology. + Experience with families/caregivers and patients through death/dying issues. Duties & Responsibilities: + Coordinates the assessment and delivery of grief counseling needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers. + Assures spiritual assessment of patients and families/caregivers in the hospice program. + Provides spiritual care to patients, families, and caregivers. + Facilitate the development of the individualized care plan. + Plans for Bereavement Program development, expansion, and refinement through annual evaluation of services. + Provides bereavement support to hospice personnel coping with work-related grief through one-to-one counseling, supervision of interns providing one-to-one counseling, and referral to community resources. + Provides bereavement information and referral services to callers from the community. Expands and maintains community resource list as needed. + Assists as needed in evaluating and planning services to meet hospice personnel support needs (i.e., yearly retreats, weekly support groups). + Functions independently but seeks and accepts guidance from other members of the interdisciplinary group, or from member of the community. + Participates in hospice activities such as Inservice education, hospice personnel meetings, and relevant committees. + Advances professional knowledge and skills. + Actively participates in quality assessment performance improvement teams and activities.
