As a Hospice CNA, you will:
Patient Preparation & Communication: Familiarize yourself with care plans, check in with patients, and provide clear, compassionate communication to patients, families, and staff in the Genesee County area.
Direct Patient Care: Support patients with personal care, mobility, and comfort needs, including safe transfers using equipment like Hoyer lifts and wheelchairs. Assist with meals and hydration as directed.
Monitoring & Reporting: Observe and report any changes in patient condition to the RN promptly, ensuring care plans are updated to reflect patient needs.
Emergency Response: Handle emergencies (e.g., falls, seizures, bleeding) calmly and ensure patient safety.
Team Coordination: Collaborate with the hospice care team, families, and local contacts to provide high-quality, seamless care.
Documentation & Privacy: Maintain accurate records of patient care and adhere to HIPAA guidelines to ensure privacy.
This role requires travel within Genesee County, participation in on-call rotations, and a compassionate, self-motivated approach to working with individuals in end-of-life care.
What You’ll Need to Succeed:
Active CNA license in Michigan.
A positive attitude and commitment to quality patient care.
Strong communication skills and reliability.
A passion for going above and beyond for patients.
Reliable transportation to fulfill job responsibilities in Genesee County.
