We are currently looking for a Registered Nurse to join as a Hospice Hospital Liaison to join our team on a full time basis. The position would be responsible for assessing patients for hospice needs and transition of service and coordinating total care for hospice patients and their families.
Responsibilities Assesses the needs of the patients and provides counseling to patients and their families and coordinates the transition to hospice to promote continuity of careEstablishes and maintains liaison with physicians, external and internal customersAssists in managing and monitoring the quality and appropriateness of the services and programsConducts community outreach activities to improve community access to hospice careAssists in developing and implementing a discharge program at one of our partner hospitals.Adheres to agency infection control and safety policiesAdheres to the organization’s policy in regards to absenteeism and appearanceOmission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position Qualifications Bachelor’s degree in Nursing preferredActive New Jersey RN License is requiredExcellent clinical skills and experience in hospice nursing and community health nursingExcellent verbal and written communication skillsExcellent customer service, organizational and interpersonal skillsStrong computer skills with proficiency in MS Office SuiteWorking Conditions/Physical Demand: Fast paced health care environment with phone, scanner, email, fine motor hand movement and computer use. Use of personal automobile with valid insurance and driver’s license
