Our Hospice Liaison is responsible for assessing, planning and developing referrals. This position is located at Stony Brook Hospital in Stony Brook, NY. This position will establish and maintain professional relationships with key physician groups and referral sources, as well as act as a liaison to referral sources on behalf of Good Shepherd Hospice and identify potential opportunities for market growth and program expansion. Job Details The ideal candidate will possess excellent interpersonal, marketing and customer service skills, and act in a professional and supportive manner at all times. The Hospice Liaison is responsible for: Reviewing patient referrals, discuss hospice care options with patients and family members, and plan with both the patient and the facility to prepare for hospice care. Develops a positive working relationship with doctors, nursing facilities, and hospitals who refer clients to hospice. Coordinate discharges Execute and follow up on referrals Advocate for the patient and family and why they should chose Catholic Health Act as a liaison to referral sources on behalf of Catholic Home Care and Good Shepherd Identify potential opportunities for market growth and Hospice program expansion Minimum Requirements: Position Qualifications: Current and valid NY State RN License Acute Care hospital Experience ( minimal 2 years) Excellent interpersonal, marketing and customer service skills Two years Hospice/Home Care experience preferred Every other weekend requirement Strong computer and EMR experience.
