Your Role as a Hospice LMSW

As a Hospice Licensed Master Social Worker, you’ll provide emotional, social, and practical support to patients and families, ensuring comfort and dignity during their hospice journey.

Key Responsibilities:

Patient Assessment: Conduct comprehensive evaluations to identify the psychosocial, financial, and environmental needs of hospice patients.Plan of Care Development: Collaborate with the interdisciplinary team to create and implement personalized care plans tailored to the patient’s unique needs.Support & Counseling: Provide counseling and guidance to patients and families, addressing emotional and practical concerns, and facilitating coping strategies.Communication: Share significant findings and updates with physicians, supervisors, and care teams, ensuring seamless care coordination.Safety Advocacy: Identify and address unsafe conditions, ensuring the safety and well-being of patients in their care settings.Education: Empower patients and families by providing education about available resources, coping mechanisms, and care expectations.Documentation: Maintain accurate and timely records, including physician orders, care plans, and clinical notes.

What You’ll Need to Succeed

Education: Master’s or Doctoral Degree in Social Work from an accredited program by the Council of Social Work Education.Experience: At least 1 year of experience in a healthcare setting, preferably in hospice or home health.Licensure: Current state license as a Social Worker (LMSW).Skills: Strong verbal and written communication abilities, with a compassionate and patient-focused mindset.
