Hospice Medical Director - contingent
The Hospice Medical Director is responsible for the overall medical direction of patient and family services at Trinity Health at Home (THAH). This role involves collaborating with leadership to plan and evaluate clinical services, providing input on the QAPI and CPI committees, and leading provider relations efforts. The Director oversees the quality of medical services, ensures 24/7 physician coverage, and supports the interdisciplinary team in patient care planning and certification.
Key Responsibilities:
Lead medical care for hospice patients and oversee quality of services.Ensure compliance with state and federal regulations for hospice admissions and certifications.Collaborate with the interdisciplinary team on patient care and treatment plans.Mentor and support hospice physicians and community healthcare providers.Review and update policies related to medical services.Participate in Executive, QAPI, and Ethics Committee meetings.Provide on-call physician coverage and ensure daily visits for inpatient hospice patients.Lead educational initiatives for physicians on hospice care.Qualifications:
MD or DO licensed in the state(s) of practice.Ability to drive and possess a valid driver’s license.Strong leadership and communication skills.Experience in providing patient-centered care, including psychosocial and spiritual aspects.Ability to work autonomously and manage time effectively.REQURIED
Board certification in Hospice & Palliative Medicine or commitment to obtain.Hospital credentialing in the hospice service area.Position Highlights and Benefits:
